Vulkan: Simplify MultiFenceHolder and managing them (#4845)

* Vulkan: Simplify waitable add/remove

Removal of unnecessary hashset and dictionary

* Thread safety for GetBufferData in PersistentFlushBuffer

* Fix WaitForFencesImpl thread safety

* Proper methods for risky reference increments

* Wrong type of CB.

* Address feedback
